This is a Form of Statutory Declaration dated 2nd September, 1948 at the Commissioner for Oaths in Malacca, Malaysia. The oath is taken by Siew Tien Wang, father of Mr. Wang Tso to declare that his son’s birth date, place of birth and that he has been living in Malaya for over ten years.
