Form of affidavit to be used in lieu of a passport (Chua Kwee Sing)

This is the front and back view of the form of affidavit of Chua Kwee Sing, dated 9th June 1950. It is used in the absence of a passport. The front view contains a photograph of Chua and her personal information, such as occupation, address etc. She was applying for a 6-month visa to Indonesia to visit relatives. The second page shows various visa stamps from the Indonesian authorities.
